Cryo Freeze Cryotherapy and Recovery LLC, d/b/a CryoFreeze Sports Recovery Center
This Privacy Policy explains how Cryo Freeze Cryotherapy and Recovery LLC, an Indiana limited liability company doing business as CryoFreeze Sports Recovery Center ("CryoFreeze," "we," "us," or "our"), collects, uses, and shares information about you.
It applies to our website, our booking and intake forms, our text and email communications, our phone line, and information you give us in person at our facility at 8333 Wicker Ave, Saint John, Indiana 46373-8879.
Some calls to our phone line are answered or assisted by an automated voice agent, which identifies itself as an AI at the start of the call. Calls may be recorded, transcribed, and stored. If you mention an injury, a symptom, or an area you want to work on, that information may be logged on your contact record so our staff can help you when you arrive.
If you would rather not be recorded, tell us and we will take your details another way — text us at (219) 558-0486 or use the booking form on our website.
CryoFreeze is not a medical provider and is not a HIPAA covered entity. We do not diagnose, treat, or bill insurance for medical care.
That means the injury, soreness, and wellness information you share with us is not "protected health information" under HIPAA, and HIPAA's protections do not apply to it. We describe it here plainly so you can decide what you are comfortable telling us.
We treat this information as sensitive regardless. We use it only to deliver your session safely, to recommend which therapies may suit what you are working on, and to keep a record of your visits. We do not sell it, and we do not use it for advertising targeting.
Please do not send us detailed medical records, diagnoses, or test results. If a therapy may interact with a medical condition, talk to your doctor.
We use text messaging in two distinct ways, and they are treated differently:
Consent to receive marketing texts is not a condition of purchasing anything from us.
Message frequency varies. Message and data rates may apply. Reply STOP to any message to opt out of marketing texts, or HELP for help. You can also call or text us at (219) 558-0486. Opting out of marketing does not stop transactional messages about an appointment you have booked.
Mobile opt-in data and consent records are not sold or shared with third parties for their own marketing.
Our website uses cookies and similar technologies to make the site work, remember your preferences, understand how the site is used, and measure advertising.
You can control cookies through your browser settings. You can also limit ad personalization through the settings of the relevant advertising platform. Blocking some cookies may affect how the site works.
We do not currently respond to browser "Do Not Track" signals, as no common standard has been adopted.
We do not sell your personal information.
We share information only as described here:
Our services are available to guests from 6th grade and above. A parent or legal guardian must create the profile in the minor's name and must sign the consent in their own name.
Our website is not directed to children under 13, and we do not knowingly collect personal information online from a child under 13. If you believe a child under 13 has given us information, contact us at [email protected] and we will delete it.
A parent or guardian may request access to, correction of, or deletion of their minor's information by contacting us.
We keep information for as long as needed for the purpose it was collected, and then for as long as reasonably necessary for legal, tax, accounting, insurance, and dispute-resolution purposes.
Whatever state you live in, you can:
To make a request, email [email protected] or call (219) 558-0486. We may need to verify your identity before acting. We will respond within a reasonable time and, where a law sets a deadline, within that deadline.
Indiana residents. The Indiana Consumer Data Protection Act took effect January 1, 2026. It applies to businesses handling the personal data of at least 100,000 Indiana residents in a year, or 25,000 Indiana residents where more than half of gross revenue comes from selling personal data. We are a single-location business and do not meet those thresholds, so the Act does not currently apply to us. We honor the access, correction, and deletion requests described above regardless.
We use reasonable administrative, technical, and physical safeguards appropriate to the size of our business. Card payments are handled by PCI-compliant payment processors, and full card numbers are not stored on our systems.
No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
If a breach of your personal information occurs, we will notify you and the Indiana Attorney General as required by Indiana's data breach notification law.
Our site links to third-party sites and tools, including our payment provider, our booking calendar, our massage therapist's booking page, and our social media pages. This policy does not cover those sites. Review their privacy policies before providing information.
We may update this policy. When we do, we will change the "Last updated" date at the top. If changes are significant, we will take reasonable steps to notify you. Continuing to use our services after an update means you accept the revised policy.
