1. Who We Are

This Privacy Policy explains how Cryo Freeze Cryotherapy and Recovery LLC, an Indiana limited liability company doing business as CryoFreeze Sports Recovery Center ("CryoFreeze," "we," "us," or "our"), collects, uses, and shares information about you.

It applies to our website, our booking and intake forms, our text and email communications, our phone line, and information you give us in person at our facility at 8333 Wicker Ave, Saint John, Indiana 46373-8879.

2. Information We Collect

Information you give us

• Contact and account details — name, email address, phone number, and, for a minor, the name and contact details of the parent or guardian who created the profile.
• Booking information — the services you select, appointment dates and times, visit history, and membership status.
• Payment information — processed by our payment providers. We do not store full card numbers on our own systems. We may retain the last four digits, card type, and a transaction record.
• Intake and consent information — the one-time electronic consent completed before your first visit, and anything you choose to tell our staff about injuries, soreness, prior surgeries, medical conditions, or what you are working on.
• Messages you send us — texts, emails, form notes, and voicemails.

Information collected automatically

• Device and usage data — IP address, browser and device type, pages viewed, referring page, and time on site.
• Cookies and similar technologies — including advertising and analytics pixels. See Cookies & Tracking.

Phone calls and our AI phone assistant

Some calls to our phone line are answered or assisted by an automated voice agent, which identifies itself as an AI at the start of the call. Calls may be recorded, transcribed, and stored. If you mention an injury, a symptom, or an area you want to work on, that information may be logged on your contact record so our staff can help you when you arrive.

3. About Health-Related Information

We treat this information as sensitive regardless. We use it only to deliver your session safely, to recommend which therapies may suit what you are working on, and to keep a record of your visits. We do not sell it, and we do not use it for advertising targeting.

Please do not send us detailed medical records, diagnoses, or test results. If a therapy may interact with a medical condition, talk to your doctor.

4. How We Use Information

• To schedule, confirm, remind you about, and deliver your appointments.
• To process payments, memberships, and recurring billing.
• To operate your membership, including visit counts and renewals.
• To answer your questions and provide customer support.
• To send you service messages — booking confirmations, reminders, changes, and receipts.
• To send marketing messages, where you have given the required consent (see Text Messaging).
• To keep the facility safe and to comply with our consent and waiver requirements.
• To measure how our website and advertising perform.
• To meet legal, tax, accounting, and recordkeeping obligations.

5. Text Messaging (SMS)

We use text messaging in two distinct ways, and they are treated differently:

• Transactional texts — booking confirmations, appointment reminders, schedule changes, and replies to your questions. These relate to an appointment you booked.
• Marketing texts — offers, promotions, and news. We send these only if you gave prior express written consent, typically by checking a clearly labeled box that is separate from booking.

Consent to receive marketing texts is not a condition of purchasing anything from us.

Message frequency varies. Message and data rates may apply. Reply STOP to any message to opt out of marketing texts, or HELP for help. You can also call or text us at (219) 558-0486. Opting out of marketing does not stop transactional messages about an appointment you have booked.

Mobile opt-in data and consent records are not sold or shared with third parties for their own marketing.

6. Cookies & Tracking

Our website uses cookies and similar technologies to make the site work, remember your preferences, understand how the site is used, and measure advertising.

• Essential — needed for the site, forms, and booking widgets to function.
• Analytics — help us understand which pages people use.
• Advertising — we may use advertising pixels, including the Meta (Facebook and Instagram) pixel, to measure ad performance and show ads to people who have visited our site.

You can control cookies through your browser settings. You can also limit ad personalization through the settings of the relevant advertising platform. Blocking some cookies may affect how the site works.

We do not currently respond to browser "Do Not Track" signals, as no common standard has been adopted.

7. How We Share Information

We do not sell your personal information.

We share information only as described here:

• Service providers who operate parts of our business under contract — our payment processor, our booking and customer messaging platform, email and SMS delivery providers, analytics and advertising platforms, and our website host.
• Our massage therapist, who books separately through her own system. When you book with her, her own booking provider handles that information.
• Professional advisors — accountants, insurers, and attorneys, where reasonably needed.
• Legal and safety — when required by law, subpoena, or court order, or where we believe disclosure is necessary to protect the rights, property, or safety of any person.
• Business transfer — if the business is sold, merged, or reorganized, information may transfer as part of that transaction.

8. Children and Minors

Our services are available to guests from 6th grade and above. A parent or legal guardian must create the profile in the minor's name and must sign the consent in their own name.

Our website is not directed to children under 13, and we do not knowingly collect personal information online from a child under 13. If you believe a child under 13 has given us information, contact us at [email protected] and we will delete it.

A parent or guardian may request access to, correction of, or deletion of their minor's information by contacting us.

9. How Long We Keep Information

We keep information for as long as needed for the purpose it was collected, and then for as long as reasonably necessary for legal, tax, accounting, insurance, and dispute-resolution purposes.

• Booking and membership records — kept while you are a customer and for a period afterward for accounting and liability purposes.
• Signed consents and waivers — kept for the period our insurer and counsel advise, which is typically longer than the customer relationship.
• Call recordings and transcripts — kept for a limited operational period and then deleted.
• Marketing contact records — kept until you opt out, and a suppression record kept afterward so we do not contact you again.

10. Your Choices and Rights

Whatever state you live in, you can:

• Ask us what information we hold about you.
• Ask us to correct information that is wrong.
• Ask us to delete information, subject to records we are required or permitted to keep.
• Opt out of marketing emails using the unsubscribe link, and marketing texts by replying STOP.

11. How We Protect Information

We use reasonable administrative, technical, and physical safeguards appropriate to the size of our business. Card payments are handled by PCI-compliant payment processors, and full card numbers are not stored on our systems.

No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

If a breach of your personal information occurs, we will notify you and the Indiana Attorney General as required by Indiana's data breach notification law.

12. Other Websites

Our site links to third-party sites and tools, including our payment provider, our booking calendar, our massage therapist's booking page, and our social media pages. This policy does not cover those sites. Review their privacy policies before providing information.

13. Changes to This Policy

We may update this policy. When we do, we will change the "Last updated" date at the top. If changes are significant, we will take reasonable steps to notify you. Continuing to use our services after an update means you accept the revised policy.
