1. Agreement to These Terms

These Terms of Service ("Terms") govern your use of the website, booking tools, facility, and services of Cryo Freeze Cryotherapy and Recovery LLC, an Indiana limited liability company doing business as CryoFreeze Sports Recovery Center ("CryoFreeze," "we," "us," or "our").

By booking an appointment, purchasing a package or membership, or using our facility, you agree to these Terms. If you do not agree, do not use our services.

2. Who May Use Our Services

You must be at least 6th grade age to use our therapies.

Guests under 18. A parent or legal guardian must create the profile in the minor's name and sign the consent in the parent or guardian's own name. By doing so, that adult confirms they have authority to consent on the minor's behalf and accepts these Terms for them.

You must be 18 or older to purchase a membership or enter into a recurring billing arrangement.

You confirm the information you give us is accurate, and that you will tell us about any condition that may make a therapy unsafe for you.

3. Our Services

We provide non-medical active recovery and wellness services, which currently include full-body cryotherapy, infrared sauna, compression therapy, hydromassage, spinal decompression, contrast therapy, and TENS/EMS. Massage, cupping, and stretch therapy are provided by our in-house massage therapist and are booked separately through her own system.

Services, equipment, pricing, and hours may change. The current offering and pricing are those shown at our facility and on our website at the time of booking.

4. Booking, Arrival, and Cancellation

• Appointments may be booked online, through the Square Go app, or by calling or texting us.
• The last appointment of the day is 15 minutes before closing. We reserve the right to decline service to anyone arriving after that time without an appointment.
• Walk-ins are welcome, but availability is not guaranteed.
• When booking multiple therapies in one visit, the order you select them affects availability. The sauna must be the last therapy of a visit.
• Sessions begin at the appointment time, not on arrival. Arriving late may shorten your session.
• Please tell us as soon as possible if you need to reschedule, so the slot can be released.

We may cancel or reschedule an appointment because of equipment issues, staffing, weather, or safety. If we do, we will offer you another time.

5. Free First Visit

New guests may be offered a complimentary first visit. It is limited to one per person, is not transferable, has no cash value, and cannot be combined with other offers. We may require the consent form to be completed before the visit takes place. We reserve the right to verify eligibility and to decline duplicate claims.

6. The 10-Day Reset

The 10-Day Reset is a one-time promotional package. Its terms are:

• A one-time payment of $49.
• Three visits in total, to be used within ten days of your first visit.
• Two recovery therapies of your choice on each visit — any two of cryotherapy, compression therapy, and hydromassage therapy.
• Limited to one per person. Not transferable and no cash value.
• Unused visits expire at the end of the ten-day window and are not refunded or extended, except at our discretion.
• If you join a membership after completing the Reset, the full $49 is credited toward your first month. This credit is applied at the time you join and cannot be redeemed for cash.

Promotional terms in effect at the time of your purchase govern that purchase.

7. Memberships and Recurring Billing

• Membership benefits, visit allowances, and rates are those disclosed at the time you join.
• Billing recurs on the same schedule each period until cancelled.
• Unused visits do not roll over to the next period unless the membership terms expressly say otherwise.
• Memberships are personal to you and are not transferable or shareable, except under a Family plan for the members named on it.
• We may change membership pricing or benefits on reasonable advance notice. If you do not accept the change, you may cancel before it takes effect.

Cancelling

You may cancel by contacting us in writing — email [email protected] or text (219) 558-0486. Cancellation takes effect at the end of the current billing period. Charges already billed are not refunded, and you keep access for the remainder of the period you paid for.

8. Payment, Pricing, and Refunds

We say this plainly rather than burying it, because we would rather you know before you buy than after.

• Prices are as displayed at the time of purchase and may change.
• Where a promotion offers a credit toward a membership, that credit is applied to a future charge and is not paid out in cash.
• We accept FSA and HSA payments where permitted. You are responsible for confirming eligibility with your plan administrator, and we make no representation that any service qualifies.
• Payment is processed by third-party providers subject to their own terms.
• If a payment fails, we may suspend membership access until it is resolved.

9. Health, Safety, and Assumption of Risk

You agree that:

• You will complete our consent form truthfully and disclose conditions relevant to your safety.
• You have consulted a physician if you have any condition that may be affected by extreme cold or heat, electrical stimulation, compression, or spinal traction — including but not limited to pregnancy, cardiovascular conditions, uncontrolled blood pressure, circulatory disorders, neuropathy, seizure disorders, implanted electronic devices such as pacemakers, recent surgery, open wounds, or cold-related conditions.
• You will follow all instructions from our staff, including time limits, protective gear, and positioning.
• Face mask and ear coverings are required for cryotherapy. Sets are available for purchase or you may bring your own, and they must be brought to each cryotherapy visit.
• You will stop a session and tell staff immediately if you feel unwell, numb, dizzy, or in pain.
• You will not use our services under the influence of alcohol or drugs.
• You will wait at least one hour after training before using active recovery services.

You assume the inherent risks of participation. We may refuse or stop a service at any time if we believe it is unsafe.

11. Facility Rules and Conduct

• Shoes are removed at the door.
• Recommended attire is sportswear — shorts or joggers and a shirt that covers the armpits. A tank or sports bra may be worn underneath for cryotherapy and sauna.
• Please arrive clean. Do not arrive immediately after training without allowing time to cool down.
• Treat staff, other guests, and equipment with respect.
• Do not use equipment without instruction, and do not exceed set time or intensity limits.

We may refuse service, end a session, or cancel a membership without refund for behavior that is unsafe, abusive, or damaging to property.

12. Photography and Media

We sometimes photograph or film in the facility for marketing. We will ask for your written permission before using any image or recording in which you are identifiable, and for a minor, from a parent or guardian. You may decline, and you may withdraw permission for future use by contacting us.

13. Intellectual Property

The CryoFreeze name, logo, website content, photography, and materials are owned by us or our licensors and may not be copied or used without written permission. You may view and print pages for your own personal, non-commercial use.

14. Third-Party Services

Our booking, payment, messaging, and mapping tools are provided by third parties and are subject to their own terms and privacy policies. Our massage therapist books through her own separate system, and her services are provided under her own terms. We are not responsible for third-party services.

You agree to indemnify and hold harmless Cryo Freeze Cryotherapy and Recovery LLC and its owners, employees, and contractors from claims, damages, and reasonable legal costs arising from your breach of these Terms, your misuse of our services or facility, or your failure to disclose a condition affecting your safety.

18. Governing Law and Disputes

These Terms are governed by the laws of the State of Indiana, without regard to conflict-of-law rules.

Any dispute will be brought exclusively in the state or federal courts located in Lake County, Indiana, and you consent to the jurisdiction of those courts.

19. Changes to These Terms

We may update these Terms. The "Last updated" date will change when we do. Continuing to use our services after an update means you accept the revised Terms. Purchases are governed by the Terms in effect at the time of purchase.

20. General

If any provision of these Terms is found unenforceable, the rest remains in effect. Our failure to enforce a provision is not a waiver of it. These Terms, together with your signed consent and any purchase-specific terms, are the entire agreement between us. You may not assign these Terms; we may assign them in connection with a sale or reorganization of the business.
